Shortcuts to save money? - the cost-effectiveness of different approaches towards dental caries.

Not removing carious tissues, i.e. sealing, infiltrating or selectively excavating caries lesions, has been criticised as “shortcuts to save money”. However, a growing number of studies have not only found less invasive approaches towards caries to be potentially less costly than the traditional approach, but also clinically beneficial. This presentation will outline current evidence, but will also demonstrate the great lack of knowledge in many areas associated with caries sealing or removal. It will then reach beyond clinical data and introduce further outcome parameters besides success or survival, one of them being efficiency. Based on a variety of studies, the cost-effectiveness of leaving versus removing caries tissues will be evaluated. Eventually, costs, clinical benefits and further outcomes will be balanced against each other.

Falck Schwendicke graduated in 2008 as dentist. Currently he is Associate Professor, Department of Operative and Preventive Dentistry, Charité – Universitätsmedizin Berlin, Germany. His research interest is focussed on minimal and micro-invasive treatment of caries lesions, evidence-based dentistry and public health and epidemiology. Falck Schwendicke has received several prizes for his work.

SEAL-DK: Sealing of manifest occlusal caries lesions in permanent teeth

In the SEAL-DK project, the possibility of treating manifest occlusal caries with non-invasive sealing instead of conventional resin composite restorations is investigated.
SEAL-DK is a randomised clinical study and includes 521 occlusal caries lesions in 521 patients aged 6-17 years. The presentation will address the options for extending the current criteria for non-invasive sealing from initial to manifest occlusal caries lesions in the permanent dentition. However, the main challenge in fissure sealing is the need for re-treatments mostly due to inadequate retention. The topic focuses also on how to make adequate resin sealing restorations with high longevity and efficacy. 
Azam Bakhshandeh graduated at the School of Dentistry, University of Copenhagen in 2005 and obtained a PhD degree in the field of Cariology with focus on diagnostic and treatment of occlusal caries lesion in permanent dentition in 2011.
Azam is Assistant professor in department for Cariology and Endodontics, Pediatric dentistry, and Radiology at the University of Copenhagen.

2. Tooth wear: Minimally invasive restorations.

Restorative treatment of severe tooth wear: from direct techniques to CADCAM.

Bas Loomans

The restorative treatment of patients with severe tooth wear should be adhesive and minimally adhesive especially in patients who have already have lost a considerable amount of tooth tissue. In his presentation Dr. Bas Loomans will show some results of a large clinical trial on restorative treatment of tooth wear patients. Minimally invasive techniques that will be discussed include direct and indirect composite restorations as well as advanced CADCAM ‘tabletop’restorations, all made to an increased vertical dimension.

Bas Loomans graduated as a dentist in 2002 and completed his PhD on proximal contact points in 2007. Nowadays he is assistant professor and principal researcher in the Nijmegen tooth wear project at the Radboud University. He also works as a dentist in a private practice.
